• No payment information is requested.
• No student login or learning record is created through this form.
• Waitlist information is used to respond to the interest type selected on the form.

To request removal from the waitlist records, contact Polus Education using the email address connected to your submission. Additional privacy terms will be added before any student accounts, paid access, or learning-data collection begins.
